Technology Integration Launches with First Joint Customer of Relevant Healthcare and CareMessage

In 2022, Relevant and CareMessage developed an integration that allows health centers to automatically enroll patients in a CareMessage Program, based on dynamic audience criteria defined in Relevant’s analytics and population health platform. These Programs are tailored to provide automated communication with patients on key areas that impact health outcomes, such as closing gaps in care, addressing social determinants of health, and gathering patient satisfaction data.
CrescentCare, a Federally Qualified Health Center (FQHC) serving patients in Greater New Orleans and Southeastern Louisiana, is the first joint customer of Relevant and CareMessage to launch a new Programs integration. Via this integration, a random sampling of patients who speak with CrescentCare’s call center will automatically receive Patient Satisfaction Surveys via text message.
On a daily basis, Relevant will utilize the CareMessage application programming interface (API) to send an updated list of patients who qualify for the survey. For staff at CrescentCare, this means the program will continue running automatically for as long as they like, with no manual steps required.
