Community of Hope Receives CareMessage Impact Fund Champion Grant to Further Health Equity Progress in Washington, DC
Washington, DC, June 10, 2024 — Community of Hope will be the recipient of a $10,000 Champion Grant as part of the CareMessage Impact Fund.
The CareMessage Impact Fund Champion Grant is provided to organizations which are best established to drive health equity in their region through the use of CareMessage, and to champion population health best practices. To date, Community of Hope has had success in using the CareMessage platform to achieve key operational and clinical goals, including, but not limited to: encouraging cancer screenings, following up with patients with recent Emergency Room visits, providing new parent resources, and helping patients access Medicaid recertification and other services and resources. This grant is intended to support future work by Community of Hope in targeting groups of patients with chronic diseases and screening needs, helping to deliver reminders and effectively prompt patient action with CareMessage Programs that simplify messaging and staff workflow.

We provide healthcare, housing, and supportive services to make Washington, DC more equitable. We operate three community health centers, a community resource hub, a Family Success Center. Our health services include medical, dental and emotional wellness as well as extensive patient supportive services for 15,785 patients last year. We work to end and prevent homelessness by providing housing and supportive services such as employment search and training, housing search, and youth services for 1,728 families and individuals. A Family Success Center connects over 500 people a year to resources.
