2024 Findings on SDOH Access Amongst Low-Income Populations
CareMessage is focused on improving Health Equity. We recently released our updated health equity framework with the three core pillars we focus on to measure improvements in health equity:
- Increasing Access to Care
- Improving Clinical Outcomes
- Addressing Social Drivers of Health
These three dimensions work in sync to drive measurable improvements in health equity. The framework was informed by insights from our customers, partners, and other experts. However, one vertical in particular was missing the voice of our target populations, social drivers of health.
Even though various value-based care reimbursement models incentivize addressing social drivers of health (SDOH), why do gaps still exist that lead to patients having worse outcomes? This summer we set out to answer this critical question. Our user research allowed us to hear directly from our target populations on the ways they access social drivers of health resources.
Although this research was initially meant to inform internal strategy decisions, we felt it would be valuable to share it more broadly. Our findings shifted how we think about the catalysts and barriers to accessing SDOH resources. We hope this will challenge how others understand these barriers and think about innovative approaches to meeting the needs of low-income populations.
